Pre Participation Physical Form

All Mercy athletes are required to have a Mercy Pre-Participation Physical Exam Form uploaded in the Magnus Health Portal prior to clearance for participation. No other forms or physician notes will be accepted. We recommend bringing the form to the student’s appointment for the physician or health care provider to complete. Physicals are valid for 365 days from the date signed by the physician. We highly recommend physicals are completed between June 1 and July 8 for the student to by fully eligible through the entire school year and avoid having to schedule an appointment mid year. All athletes must have a physical on file to be eligible to participate in any Summer Strength & Conditioning sessions and open gyms.

The Mercy Pre-Participation Physical Exam Form must be completed by either a physician (MD or DO), Physician Assistant (PA), or Nurse Practitioner (NP). All physical forms must be dated, signed, and stamped by the health care provider. The form will not be valid if it is not dated, signed, and stamped.
